YOUR VISIT
NEW PATIENT VISIT
DURATION: up to 90 minutes
COST: $550 (payment in full is required at the time of visit)
INCLUDES: Review of full medical history, previous labs, previous imaging, physical examination, evaluation of current medications/ supplements, personalized treatment plan.
EXTENDED FOLLOW UP
DURATION: up to 60 minutes
COST: $395 (payment in full is required at the time of visit)
INCLUDES: Review of 2 or more new lab results, track your progress, make adjustments to your treatment plan if needed.
FREQUENCY: Follow-up visit frequency depends on the type of labs ordered, treatment plan and doctor’s recommendation. Some patients will require more frequent follow-ups.
FOLLOW UP
DURATION: up to 30 minutes
COST: $195 (payment in full is required at the time of visit)
INCLUDES: Review of 1 new lab result, track your progress, make adjustments to your treatment plan if needed, reassess if you haven’t seen the doctor in 6 months or longer.
FREQUENCY: Follow-up visit frequency depends on the type of labs ordered, treatment plan and doctor’s recommendation. Some patients will require more frequent follow-ups.
QUICK FOLLOW UP
DURATION: up to 15 minutes
COST: $85 (payment in full is required at the time of visit)
INCLUDES: Review of 1 new chief complaint (acute) or review of simple lab results.
LAB TESTING
Labs are not covered by insurance at this time.
*NOTE: We work with several specialty labs with fair pricing for people without insurance. The necessity for these labs will be discussed with the doctor.
INSURANCE
We don’t accept any insurance.
Our office is not in network with any insurance provider.
*NOTE: if you have PPO, please contact your provider and find out if your visit is eligible for partial or full reimbursement. Some PPO providers offer an out of network coverage.
PAYMENT
Payment in full is required at the time of each appointment or service.
We accept cash, debit, and all major credit cards.
